Vaccinations required for incoming Kindergarten students!

The nurse will need proof of vaccination. So, you can either bring a shot record with them on their first day of school or fax it to the office. Fax #: 940-276-1564 

If your student is exempt from vaccinations you are required to provide an exemption form from the state of Texas that is signed and notarized. If you have any questions please contact me. Thanks!
