Athletic/Band/Cheer Physicals

Parents, I will be sending home the Physical’s paperwork with your child today/tomorrow. This is for 6th graders going into 7th, 8th graders going into 9th, and 10th graders going into 11th (or if you’ve had a major injury). If your child falls into one of these categories and you did not receive the paperwork you can get it at https://www.uiltexas.org/files/athletics/forms/PrePhysFormRvsd1_10_20.pdf . We will be doing PRE physicals on Monday, paperwork needs to be turned in prior to or on Monday. The actual physicals (with the doctors) on Wednesday May 7th at 3:30pm. All students (incoming 7th, 9th & 11th graders or anyone who was held from participation by a doctor) in athletics, marching band and cheer must have a current physical. I have also attached the form as well. 6th graders (and Parents) wanting to be in athletics next year need to join the SportsYou group (with the code JKKP-7P3R) so you can be involved this summer with camps, summer workouts, etc.
